Take Charge! – Guided Advance Care Planning


What matters most to you? If you were facing serious illness, what kind of care would you want? If you couldn’t speak for yourself, who would?

Planning for your future care can bring peace of mind – to you and your loved ones. Advance care planning is a process that helps you decide and document what kind of care you would want – and what kind you would not want – if you have a sudden or serious illness diagnosis and are not able to communicate or make decisions.

In this free online class, you will learn more about advance care planning and the steps you can take to help get the care you want, based on what matters most to you. This workshop is appropriate for people newly considering this topic and for people who want to update their documentation.

We will cover the basics of advanced care planning – how to think about it, talk about it with loved ones and health care professionals, how to choose a health care agent, and the appropriate forms, as well as special circumstances, such as a dementia directive, End of Life Options Act, interactive scenarios, and time for Q&A.

This class will not be recorded. Attendance requirement for incentive points – at least 80% of the live session. Request disability accommodations and access info.
