Session Description:
We will be discussing an overview of health inequities and how they relate to health outcomes in LatinX people living with diabetes, as well as innovative programs aimed to close the inequity gaps. We will discuss a novel approach aimed to improve health and decrease avoidable hospitalizations, an alternative model of care to the one-on-one medical appointment, and how telehealth has impacted this patient population.
